Supporting individuals to remain in or return to work

Bespoke pathways supporting people who are absent from work.

Our Vocational Rehabilitation Coaching Rehabilitation programmes are designed to support people that are off work with a long-term health condition. Our programmes are delivered by a specialist team of HCPC-registered clinical health coaches to actively manage a return to work through Vocational Rehabilitation Coaching sessions.

Multi and interdisciplinary active management

More complex cases for those people off work often need more time spent with their Vocational Rehabilitation Coach, or a variety of coaches, enabling them to adjust to their illness or condition and embedding behaviour change to ensure increased functioning and a sustained return to work.

Bespoke pathways

Individuals follow pathways of care depending on their diagnosis. Each pathway utilises the Vocational Rehabilitation Coaching approach to enable self-management of symptoms in the workplace.

Employer support

Where consent is given, W2W can liaise with employers regarding their employee’s support. This enables employers to better support their employee following W2W’s support.

How Vocational Rehabilitation Coaching Rehabilitation Works

As each individual’s circumstances are different, an initial assessment will take place focusing on where someone may be struggling at work.

Referral

Once onboarded, an organisation can refer an individual using a referral form. Following the referral, W2W’s clinical admin team will be in touch with the individual to book an initial assessment.

Initial Assessment

Before assessment, the individual will be asked to complete pre-assessment questionnaires, this gives the clinical health coach a better idea of how the they may be struggling. An initial assessment will then take place where the clinical health coach will gain a better understanding of the individual’s symptoms.

Report and Recommendations

Following the initial assessment, the clinical health coach will then write a report and provide recommendations for tailored follow-up support.

Clinical health coaching

Vocational Rehabilitation Coaching Rehabilitation programmes include a minimum of six hours of clinical health coaching sessions. These sessions are focused on actively managing an individual’s return to work journey with the aim of returning to work whilst also preventing future absence. Sessions are tailored based on the individual’s needs and the outcomes of their initial assessment.

Employer conversations

During support, where consent is given, the clinical health coach may speak with the individual’s employer. This may be to discuss employer support including reasonable adjustments.

Additional sessions

Additional sessions can be added onto our rehabilitation programmes where further support may be needed.

The expected outcome of this level of service is supporting the individual to a sustained return to work and increased productivity.
